Accident Summary:

 ^1LT KLASE^ ON THE MORNING OF THE ACCIDENT, TOOK OFF AT APPROXIMATELY 0700 HOURS. THREE VISUAL RECONNISSANCE BOXES WERE WORKED PRIOR TO ENTERING THE CRASH SITE BOX. APPROXIMATELY 6 HOURS OF FLIGHT TIME WAS LOGGED PRIOR TO THE ACCIDENT. ^1LT KLASE^ ENTERED THE VISUAL RECONNISSANCE BOX WHERE THE ACCIDENT OCCURRED AT APPROXIMATELY 1500 HOURS. HE FLEW THE BOX FOR 50 MINUTES BEFORE SPOTTING AN ENEMY LOCATION. AFTER RECEIVING CLEARANCE, HE ENGAGED THE AREA WITH HAND FRAGS AT TEN (10) KNOTS INDICATED AIRSPEED. HE INCREASED POWER TO GET AWAY FROM THE EXPLOSION. AT THIS TIME ROTOR AND ENGINE RPM BEGAN TO DETERIORATE, THE AIRCRAFT HIT A TALL TREE AND CONTINUED DOWN THE SIDE OF THE MOUNTAIN. HE NOTICED THE ENGINE OUT AUDIO AND ENTERED AUTOROTATION. AT THIS TIME THE PILOT NOTICED A SEVERE VIBRATION IN THE AIRCRAFT AND ASSUMED THAT IT WAS A RESULT OF HITTING THE TREE. DUE TO THE INSUFFICIENT ALTITUDE, ONLY 60 KNOTS OF AIRPEED WAS REACHED. THE AIRCRAFT WAS DECELERATED AT APPROXIMATELY 50 FEET, WITH VERY LITTLE CUSHIONING PITCH INITIATED AT THE BOTTOM. THE AIRCRAFT SLID ACROSS THE SAND BAR AND CAME TO REST IN THE WATER ON ITS RIGHT SIDE, RESULTING IN MAJOR DAMAGE TO THE AIRCRAFT.\\
