Accident Summary:

 THE AIRCRAFT WAS ON A NIGHT VISUAL RECONNAISSANCE MISSION FOR DAU TIENG PERIMETER DEFENSE. MR. MOON WAS TOLD THAT HE WOULD FLY OH-6A, 68-17208, MR MOON HOVERED FROM THEIR REVETMENT AREA AND MET CPT PARK BY HEADQUARTERS AND STARTED THEIR MISSION SHORTLY AFTER 1800 HOURS, 19 OCT, 69. THEY HAD COMPLETED TWO TRIPS AROUND THE PERIMETER WHEN THE PILOT MADE A SHARP RIGHT TURN AND THE PILOT CLAIMS THAT RPM WAS LOST AND HE FELL INTO A STEEP DIVE. THE PILOT WAS LOW-LEVEL (50 FEET AGL) AT ABOUT 50 KNOTS FORWARD AIRSPEED WHEN THE TURN WAS INITIATED. HE LOST ALTITUDE AND GAINED AIRSPEED SLIGHTLY. THE PILOT CONTINUED THE TURN AND PULLED ALL THE PITCH REMAINING IN AN ATTEMPT TO LAND SOFTLY. THE AIRCRAFT STRUCK THE GROUND WHILE IN A RIGHT TURN AND BOUNCED BACK INTO THE AIR TO A HOVER. THE AIRCRAFT WAS THEN ABOUT 100 METERS FROM RUNWAY 08 AT DAU TIENG SO THEY HOVERED TO THE RUNWAY AND THEN TO THE REVETMENT AREA. AFTER THE CREWCHIEF CHECKED THE SKIDS, THE AIRCRAFT WAS LANDED.\\

Accident Summary:

 APPROXIMATELY 1310 HOURS 8 FEBRUARY 1970, AN OH-6A, SERIAL NUMBER 68-17208, WITH PILOT AND CREW CHIEF DEPARTED DAU TIENG AIR FIELD ON A COMMAND AND CONTROL MISSION FOR THE 1ST BATTALION, 2ND INFANTRY, 1ST INFANTRY DIVISION WHO HAD TROOPS IN CONTACT WITH AN ENEMY FORCE. ENROUTE TO THE CONTACT AREA, A STOP WAS MADE AT PINE RIDGE, COORDINATES XT522588, TO PICK UP A PASSENGER, MAJ JACK KELLY. THE AIRCRAFT THEN PROCEDED TO THE CONTACT AREA. THE AIRCRAFT ARRIVED ON STATION AND THE PASSENGER BEGAN COORDINATING WITH THE COMMANDER ON THE GROUND. ALSO IN THE CONTACT AREA, COORDINATES XT 5522720, WAS ON OV-10 CONTROLLING AN AIR STRIKE. THE PILOT OF THE OH-6A, CPT VOTH, TUNED IN THE FREQUENCY THE OV-10 AND THE HIGH PERFORMANCE AIRCRAFT WERE UTILIZING AND BEGIN MONITORING THE PROGRESS OF THE AIRSTRIKE. THE OH-6A HAD BEEN ON STATION APPROXIMATELY 20 TO 25 MINUTES AND DURING THIS TIME HAD MONITORED THE PILOT OF THE OV-10 AIRPLANE TELL THE PILOTS OF THE HIGH PERFORMANCE AIRCRAFT THAT THEY WERE RECEIVING ENEMY FIRE ON THEIR PASSES. THE OV-10 AIRCRAFT THEN RECEIVED HEAVY GROUND FIRE AND THE PILOT AND PASSENGER WERE FORCED TO EJECT FROM THE CRAFT. CPT VOTH, PILOT OF THE OH-6A, FOLLOWED ONE OF THE PARACHUTING INDIVIDUALS, LATER IDENTIFIED AS FLT LT LANGTON, UNTIL HE LANDED ON THE GROUND. CPT VOTH THEN ELECTED TO TRY AND LAND IN AN EXTREMELY SMALL LANDING ZONE, THE ONLY AREA NEAR THE PILOT, AND PICK UP FLT LT LANGTON. CPT VOTH ATTEMPTED A VERTICAL DESCENT INTO THE AREA. APPROXIMATELY SIX (6) FEET FROM THE GROUND THE MAIN ROTOR BLADE STRUCK A TREE. THE AIRCRAFT THEN TURNED TO THE RIGHT, THE TAIL BOOM STRIKING A TREE AND BENDING RESULTING IN FAILURE OF THE TAIL ROTOR DRIVE SHAFT AND SUBSEQUENTLY COMPLETE ANTI-TORQUE FAILURE. THE AIRCRAFT TURNED APPROXIMATELY 270 DEGREES BEFORE THE PILOT WAS ABLE TO LAND THE AIRCRAFT IN THE LANDING ZONE.\\
